    2. A signed contract with the school for students 
        with disciplinary records

D. Foreign Students:
    1. Accomplished application form
    2. ACR and passport
    3. Authorized copy (translated to English) of F137 
        or Transcript of Records by the embassy from 
        grades I to the present.
